How it works

Your health, your way: comprehensive virtual care

Health+ offers you and your family a tailored virtual health care experience. We understand that every patient is unique, and that's why we provide a range of services customized to you and your family's individual needs.

Meet Your Health Team

Onboarding

Following your sign up, you’ll meet with our team and receive an invitation to download the Health+ platform. We’ll go over your history, goals, and hit the ground running to support you and your family's health.

Ongoing care

Depending on your region, your day-to-day care will be managed by a Doctor or Nurse Practitioner. They’re responsible for recommending our other services to you, like Patient Navigators and Counselors. Whether you need a requisition or a detailed health plan, they’re here for you.

Schedule

We offer virtual care for four days a week (with a full seven-day schedule coming soon). Plus unlimited visits, and unlimited text, email and phone support. All visits are scheduled for a minimum of half an hour.
